Patient ID Smartphone App

Our patient ID card is a smart phone application. Please install it before your visit. The app is only available in Japanese, so please use the translation function to enter your information. If you have difficulty using the app, please ask at the reception desk.

How to install the app


Download and install the software from “App Store” for iPhone users or “Google Play” for Android users.



Please enter your e-mail address, name, date of birth, and telephone number.

If you are a current patient and know your patient ID number, please enter it.

If you are a first-time visitor or do not know your patient ID number, the receptionist will provide you with your patient ID number at the time of your visit, so please enter it later.


Please read the Terms of Use and press “Register”.

A registration email will be automatically sent to your registered email address.


Please check your e-mail and press [Registration URL] in the e-mail you receive.


Open the application and press the patient ID card.

If you have already registered your patient ID number in Step 2, please present the barcode to the receptionist at the time of your visit by brightening the screen.

You can register your patient ID number from “Register/Update your membership information”. If you do not have or do not know your patient ID number, please ask the receptionist.
